The Travancore Cochin medical Council has notified to update database

The Travancore Cochin medical Council has notified the homeopathic Medical practitioners registered under the Council to update the database.

the Council has asked for daily life certificate and certificate of declaration to be submitted via E-mail . A  Google form sheet has been provided by the Council. All Registered practitioners registered under the Council are also asked to fill this form by 31st of January 2019.[28/01/2019 - LAST DATE - 31st March 2019]online colleges

This procedure is followed by other system of medicines registered under the Council.

This has been done to update the latest database so that it can be forwarded  to the central Council for registration purposes and updating the central register

The life certificate have to be produced that is Attested by a gazetted officer of the Kerala state.These certificates has to be scanned and has to be mailed in the PDF format along with the copy of latest Hologram certificate issued by the Council.

Instruction Given in  the site:

Before entering details to the proforma you have to obtain life certificate in the prescribed format given in website and email a scanned copy of it to tcmchomoeo@gmail.com . The certificate should be obtained from a gazetted officer of Kerala Government. Also scanned copies of self signed declaration (format of which is given in website) and scanned copy of your original TCMC registration certificate are to be sent via email to the address tcmchomoeo@gmail.com . (All scanned copies should be in PDF form) If you have obtained life certificate and posses Self signed declaration and original TCMC registration certificatecan fill the proforma below
